Typhoid (Widal & Typhidot) test in Banur

Typhoid is caused by Salmonella Typhi, a bacterium picked up from contaminated water or food. The Widal test is the blood test most doctors in Punjab still write first for a fever that won't settle, and Alpha Laboratory in Banur also offers the Typhidot rapid antibody test. Neither needs fasting, and one blood sample covers both.

The honest catch: the Widal test has real limits, and timing matters a lot. This page explains both. To book, call 8872524007.

What the Widal / Typhoid test checks

Widal looks for antibodies (agglutinins) against parts of the typhoid bacterium. The report lists four lines with a titre against each, such as 1:80 or 1:160:

  • TO (O antigen) and TH (H antigen) for S. Typhi
  • AH and BH for S. Paratyphi A and B, which cause paratyphoid fever

Typhidot is a rapid card test that detects IgM and IgG antibodies against an outer membrane protein of S. Typhi. IgM points to recent infection.

Both measure your body's response, not the bacterium itself. Only a culture grows the actual organism, which is why some doctors add a blood culture in the first week.

Why your doctor may order it

A typhoid test is usually ordered for fever lasting more than four or five days, especially a fever that climbs step by step, with headache, tiredness, loss of appetite, stomach pain, constipation or loose motions. Cases rise in the rainy months when drinking water is more easily contaminated.

Because early typhoid can look like dengue or malaria, doctors often order the Fever Profile, or pair the Widal with a dengue test and a malaria test.

How to prepare

No fasting. Tell us how many days you've had fever, and mention if you've had typhoid before or received a typhoid vaccine; both can raise Widal titres for a long time. If you've already started antibiotics, let your doctor know, as it can affect the result.

Sample type and report time

A few millilitres of blood from the arm, in a plain or gel tube for serum. Widal and Typhidot reports are usually ready the same day if the sample is in by about 11 am. Unwell at home? Book home sample collection or call 8872524007.

Understanding your results

Widal antibodies usually start rising only at the end of the first week of fever, so a test on day 3 can be negative even in real typhoid.

The tricky part is the cut-off. In areas where typhoid is common, many people carry low titres from past exposure. Many Indian labs treat an O titre of 1:160 or more as significant, but there's no single agreed number. A rise of four times or more between two samples taken 7 to 10 days apart is far more meaningful than one reading.

Widal can also be falsely positive after malaria, other infections or vaccination. A positive Typhidot IgM supports recent infection, but it too can cross-react. Your doctor reads these results with your symptoms and other tests before deciding anything.

FAQ

Is the Widal test or the Typhidot test better?

They answer slightly different questions. Typhidot can turn positive a little earlier and reports IgM separately; Widal gives titres that can be compared over time. Your doctor chooses based on the day of fever.

Can a typhoid test be negative if I really have typhoid?

Yes, especially in the first week or after antibiotics. That's why doctors sometimes repeat the Widal after a week or order a blood culture.

My Widal is positive but I feel fine. What does that mean?

Low positive titres are common in people who had typhoid or a vaccine in the past. A single titre without symptoms doesn't confirm typhoid. Show the report to your doctor.
