Bleeding Time & Clotting Time test in Banur

BT and CT are two old, simple bedside tests that are still written on many pre-operation slips in Punjab. Bleeding time (BT) measures how long a tiny skin prick keeps bleeding. Clotting time (CT) measures how long a drop of blood takes to clot outside the body. Together they make up the bleeding time clotting time test, often shortened to BT/CT.

What the BT/CT test checks

Bleeding time mostly reflects how well platelets and the small blood vessels work. In the Duke method, the ear lobe or fingertip is pricked with a sterile lancet and the blood is blotted with filter paper every 30 seconds until it stops. The Ivy method uses the forearm with a blood pressure cuff inflated gently on the upper arm.

Clotting time reflects the clotting proteins. In the capillary method, blood is drawn into a thin glass tube and a piece is snapped off every 30 seconds until a thread of fibrin appears.

Why your doctor may order it

  • before minor surgery, tooth extraction, circumcision or a biopsy
  • before a procedure in a small clinic where it has always been the routine check
  • a history of bleeding gums, nosebleeds or prolonged bleeding after cuts

It's worth knowing the limits. BT/CT is not very reproducible and can be normal in mild bleeding disorders. Many doctors now prefer platelet count, PT and APTT, and use BT/CT alongside them or not at all. If your surgeon has asked for it, we'll do it; the choice of tests is theirs.

How to prepare

You can eat normally. Aspirin, clopidogrel and many common painkillers (like ibuprofen and diclofenac) can prolong bleeding time for days, so tell us everything you've taken in the last week or so. Don't stop a prescribed medicine for this test unless your doctor says to.

Sample type and report time

Because both tests are timed with a stopwatch on the spot, they can't be done from a tube sent from home. They are done only at the lab on M.C. Road, so home sample collection isn't possible for this one; other tests on your slip can still be taken at home. The result is usually ready before you leave.

Understanding your results

Normal values depend on the method, and the report states which one was used. As a rough guide, Duke bleeding time is usually under about 5 minutes and capillary clotting time under about 10 minutes. The range printed on your report is the one that applies.

A prolonged bleeding time can be associated with a low platelet count, platelets that don't work well (including from aspirin), von Willebrand disease or vessel problems. A prolonged clotting time can be associated with clotting factor deficiencies such as haemophilia, or with heparin. Your doctor reads the result alongside your symptoms and other tests.
