What the LFT test checks
The name is a little misleading. Most of these numbers don't measure how well the liver works. They show different kinds of trouble.
- SGPT (ALT) and SGOT (AST) are enzymes inside liver cells. When cells are inflamed or damaged, more of them leak into the blood.
- Alkaline phosphatase comes mainly from the bile ducts and from bone.
- Bilirubin is the yellow pigment left when old red cells break down. The liver processes it and sends it out in bile.
- Albumin is made by the liver, so a low level over months can point to long-standing liver disease, though diet and kidney loss matter too.
Read together, these give a pattern rather than a single answer.
Why your doctor may order it
Jaundice is the obvious one. It is also ordered for tiredness with nausea and loss of appetite, for follow-up after hepatitis, and when an ultrasound shows a fatty liver.
Doctors repeat the liver function test during TB treatment and with other medicines known to strain the liver. Regular drinkers, people with diabetes and anyone with hepatitis B or C on record are often checked yearly. It also appears in pre-surgery work-ups.
How to prepare
No strict fast is needed for most people. If your doctor wants an overnight fast of 8 to 12 hours, come before breakfast; water is fine, tea is not.
Skip alcohol for a day beforehand and avoid a heavy gym session, since hard exercise can lift SGOT from muscle. Keep taking your medicines as prescribed and bring the list with you.
Sample type and report time
A small amount of blood is drawn from a vein in the arm into a serum tube (red or yellow cap). If we have it by about 11 am, the report is usually ready the same day as a printed copy and a PDF on WhatsApp. We can also come to you through home sample collection.
Understanding your results
Doctors look at the shape of the result. Mainly raised SGPT and SGOT points towards the liver cells. Raised ALP and direct bilirubin points more towards the bile flow. A raised indirect bilirubin on its own is often harmless, as in Gilbert syndrome.
An SGOT to SGPT ratio above 2 is commonly seen in alcohol-related liver disease. A single slightly high value is common and is often just rechecked. The range printed on your report is the one that applies, because ranges vary by lab and method. Your doctor reads the result alongside your symptoms, any scan and other tests.
